Scalp reduction involved physically removing bald scalp and stretching the surrounding hair-bearing skin to fill the gap. The goal was to immediately reduce the balding area and the number of hair grafts required. It was successful at reducing baldness, but it often led to unnatural hair growth patterns and scarring in obvious places.
Flap surgery involves repositioning a strip of hair-bearing skin from the side or back of the head to the front hairline while keeping it attached to the original blood supply. The results were immediate, but there were significant risks revolving around poor blood supply, often resulting in tissue death, unnatural results, and obvious scarring.
The first revolutionary technique in hair restoration technology was follicular unit transplantation (FUT). This procedure removes a strip of scalp from the back of the head, divides it into individual follicular units (1-4 hairs), and transplants them into the targeted thinning or balding areas. After the procedure, the transplanted hair would fall out within the first few weeks, and new, permanent growth would appear 3-4 months after, with final results after 9-12 months. FUT was successful because it produced pretty natural-looking results and a high graft yield, yet a linear scar in the scalp cut area was obviously noticeable if the patient had a short haircut.
As hair restoration became increasingly popular, a new technique emerged: follicular unit extraction (FUE). This surgical technique extracts individual follicular units directly from the scalp, typically on the back or sides of the head, using a micro-punch tool, and relocates them to balding or thinning areas. There is no scar left behind, leaving an even more natural-looking appearance than FUT, a shorter recovery period with less pain, and the versatility of allowing patients to keep short hair without the embarrassment of a scar.
